• Download and fill out Initial Visit Package forms (this will speed up registration process and decrease your waiting time)

• List of ALL medications with their dose and frequency (included in Initial Visit Package forms)
• Pertinent medical records
• MRI, CAT scans (reports and films if available) and/or laboratory test results available
