• Insurance card
• Referral (if necessary)
• Download and fill out Initial Visit Package forms (this will speed up registration process and decrease your waiting time)
• Part of this Initial Visit Package is how we use and disclose your personal information. Our policy is here for your review.
• List of ALL medications with their dose and frequency (included in Initial Visit Package forms)
• Pertinent medical records
• MRI, CAT scans (reports and films if available) and/or laboratory test results available
• Browse our SELF-ASSESSEMENT FORMS page and fill out appropriate forms
